Basic Life and AD&D

ASU Preparatory Academy provides a Basic Life/AD&D benefit in the amount of $27,000 to all eligible full-time employees.

​

The Life insurance benefit will be paid to your designated beneficiary in the event of death while covered under the plan. The AD&D benefit will be paid in the event of a loss of life or limb by accident while covered under the plan. Please remember to update your beneficiary information as needed.

Voluntary Life and AD&D

​

In addition to the employer-paid Basic Life and AD&D coverage, you have the option to purchase additional voluntary life insurance to cover any gaps in your existing coverage that may be a result of age reduction schedules, cost of living, existing financial obligations, etc. You pay the full cost through payroll deductions. Your election, however, could be subject to medical questions and evidence of insurability.

You will need to complete a Statement of Health for the Voluntary Life and AD&D insurance if:

• You elect to initially enroll in an amount over the guarantee issue amount

• You elect to increase your current amount of insurance

• You declined voluntary life during your initial eligibility period and would like to enroll this year
